At Forbes Care, our Dementia and Memory Support Program has been designed to support individuals to continue living safely, independently and meaningfully within their homes and communities for as long as possible.
Our Occupational Therapists work alongside individuals and families to understand how changes in memory and cognition are impacting everyday life and identify practical strategies to support participation, safety and independence.
This is not a one-size-fits-all program.
Every person’s experience with dementia or memory changes is different. Our role is to understand the person, their routines, environment, goals and what matters most to them, then tailor supports and recommendations to suit their individual needs.
The program is designed within a reablement framework, meaning we focus on maintaining abilities, supporting independence and building sustainable routines and strategies rather than creating unnecessary dependence on services.
