Supporting Independence Through Forbes Care’s Dementia and Memory Support Program

Changes in memory, cognition and daily functioning can impact far more than forgetfulness alone.

For many older Australians, cognitive changes can gradually affect confidence, safety, routines, participation in meaningful activities and the ability to manage everyday tasks independently. Families may also begin noticing increasing concerns around medication management, appointments, safety at home, wandering, reduced motivation or changes in emotional wellbeing.

At Forbes Care, our Dementia and Memory Support Program has been designed to support individuals to continue living safely, independently and meaningfully within their homes and communities for as long as possible.

What is the Dementia and Memory Support Program?

Our Occupational Therapists work alongside individuals and families to understand how changes in memory and cognition are impacting everyday life and identify practical strategies to support participation, safety and independence.

This is not a one-size-fits-all program.

Every person’s experience with dementia or memory changes is different. Our role is to understand the person, their routines, environment, goals and what matters most to them, then tailor supports and recommendations to suit their individual needs.

The program is designed within a reablement framework, meaning we focus on maintaining abilities, supporting independence and building sustainable routines and strategies rather than creating unnecessary dependence on services.

What can Occupational Therapy support with?

Our Occupational Therapists can assist with:

Home Safety

Changes in cognition can impact safety awareness, judgement and the ability to manage daily routines safely within the home.

Our Occupational Therapists can assess and support areas such as:

  • Falls risks
  • Kitchen and cooking safety
  • Medication routines
  • Wandering concerns
  • Fatigue and reduced sequencing
  • Home environment setup
  • Memory prompts and visual cues

We work collaboratively with individuals and families to identify practical strategies and environmental supports that improve safety while maintaining dignity and independence.

Daily Routine and Functional Support

Changes in memory and cognition can make once familiar daily tasks feel overwhelming or difficult to manage.

Occupational Therapy can support:

  • Establishing structured routines
  • Simplifying tasks and environments
  • Energy conservation strategies
  • Visual prompts and memory aids
  • Supporting participation in meaningful activities
  • Maintaining independence in self-care and household tasks
